New Patient?
If you're a new patient, please bring your insurance card and arrive
early to complete any necessary paperwork. If you have been under the
care of another physician, please bring your records or have them
delivered prior to your appointment.
Which Insurance Plans to we Accept?
We accept almost all major insurance plans including Medicare and Medicaid. Always bring your insurance card to your appointments. If you have a change in insurance, address or phone number, be sure to notify the receptionist so that the change can be entered into our system.
We accept all major credit cards for payment, such as Visa and Mastercard, and American Express. All balances are expected to be paid at the time of service.
Need to Cancel an Appointment?
Please call the office prior to your appointment to cancel. 24 hour notice is greatly appreciated. If you don’t show up for your appointment twice, you may be charged a fee, for which you are responsible since insurance will not cover it.
Running Late?
We are committed to prompt service and will work very hard, barring emergencies, to stay on time. We may ask you to reschedule if you arrive for your appointment more than 10 minutes after your scheduled appointment time. Please try to arrive 10 to 15 minutes early for your appointment to complete any paperwork associated with your visit.
Need a Prescription Renewed?
Please provide our office with at least 48 hours’ notice for all prescription refills. If you are asked to leave a message, always leave your full name, date of birth, name of the medicine(s) and dosage, as well as a contact number to return your call.
Billing Information
The financial policy set forth is to expect
payment for your services at the time your services are rendered.
Co-payments and balance dues are expected to be paid at the time of
service. We accept cash, checks and major credit cards. As a courtesy
to you, we will complete your insurance claim for you. Most major
insurances, Medicaid and Medicare are accepted.
We will provide insurance claim filing for the insurance plans that
we participate in. If there is a balance due after payment is rendered
by the insurance company, we will expect your prompt payment of that
balance.
It is your responsibility to provide us with current insurance plan
information prior to services rendered in order for accurate billing of
services to be filed.
In order to accommodate the needs and requests of our patients and
the community, we have enrolled in various managed care plans. While we
gladly provide this service to our patients, it is important for the
patient to be familiar with the guidelines of their insurance plan
requirements regarding authorizations, deductibles, co-payments and
other vital requirements.
We strive to offer our patients quality medical care and assist the
patient to receive maximum benefit from their insurance plan.
Our Patient Portal
MetroSouth Health Centers offer a free online patient portal that allows you to access information about the care you’ve received at the Centers, and to communicate with your doctor/healthcare provider. The portal is secure and private; all information is sent via an encrypted connection.
Using a computer or smartphone, you can:
- View test results
- See prescribed medications
- Review summaries of your office visits
- Request appointments and prescription refills
- Receive important reminders about your care
- Send private, secure messages to your doctor/healthcare provider and receive responses
- Review and pay bills
